"Windows was unable to complete the format" error

hello, i am trying to use a USB key, when i double click on it, it says "the disk in drive D is not formatted. do you want to format it now?" when i try to format it a get "Windows was unable to complete the format" "the disk in drive D cannot be formatted" how do i fix this? thanks.
