Strange behavior when waking from sleep mode

I have as of today encountered a problem where, when attempting to wake my PC from sleep mode, it either doesn't "wake" fully or it can't seem to detect either the monitor or TV it is connected to. When waking, either from the PC's power button, mouse left click, or keyboard key click, I can hear the PC come on as if it has woken, but nothing shows up on either display and the fans run louder than normal. The only way out of this is to force shut down via the power button on the PC - when turning the PC back on after this, there is about a 50/50 chance that either A: the PC will come on normal
