Random game crash + Monitor disconnecting

So sometime when i use my computer, while playing games my second monitor connected using HDMI will randomly disconnect and shut down any game I'm playing. It never has a crash report, or at least one i can find, which just makes it so much harder to find the problem, I've had this problem for the past 2 or so years and have tried updating everything, uninstalling display adapters and trying new cords but nothing has worked.
