I need help fixing my WIFI

My WIFI keeps disconnecting every 5 mins and I need to disable and reenable my wifi adapter every time this happens, when The wifi is gone I can't find another Wifi network, My adapter info is: Manufacturer: Realtek Semiconductor Corp.
