How to upload 3 files to a website?

Please can someone inform me where I'm going wrong as every time I fill out a form to apply for s disabled badge. When I go into documents to select a page scanned in and select the form has lost all my details and I've to start from scratch fill all personal information but 2nd and 3rd attempt it resets.
