How do I remove the authenticator program that attached itself to my Amazon account?

In the last week, I have been hacked numerous times. They hacked my bank account to the tune of $2,300, putting that money into one of my credit cards, (probably to use the credit card because they probably didn't have the proper credentials to take the money out of the bank). They also hacked 2 of my credit cards, and hacked my Amazon account twice, the second time they somehow attached an authenticator program to Amazon (I had already recaptured Amazon and changed the password. Amazon has escalated this issue up the chain to the next level for resolution.) My question is: I don't rememb
