HAS ANY ONE MADE A BUSINESS VOTER BALLOT

I am trying to create a ballot that can be used for a community vote.

I can not insert the boxes ( draw a table) to align with the policies we are

voting on.

Anyone have any ideas. Thanks.

Would be helpful to know exactly which program you're trying to use for

this. Word? Publisher? Something else? What version?
