Disk issues. 100% when launching any application.

I’m having problems with my disk - I’ve done just about every problem solving solution I can find/think of and whenever I launch any application (whether its a heavy application or not) the disk shoots to 100% and makes everything load sufferingly slow. I recently replaced my hard drive and this issue reoccured. Any help?
