Computer factory reset itself without any user input, what do I do?

I had recently factory reset my computer due to an unrelated issue, and all seemed to go well. That is, until I reset (turned it off and on again) to fix a problem - which is when it then appeared to factory reset again - without me telling it to. Is there anything I did wrong, and how can I fix this problem?
