Calendar invites randomly being sent

We're running Exchange 2010 and Office 2010, both are patched to the newest available (inc SP1 on Exch).

For the last few months there has been this very odd issue, whereby a calendar invite will be automatically sent from User A to user B without their knowledge. It is always meetings that were set up by former staff members and later canceled.

It's embarrassing for User A because they are unknowingly delivering a meeting invite for something that was already canceled to user B and it creates confusion.

The meeting requests always originate from User A, which is the same person in our organization. She's tired of having people stick their head in her office and query things that she didn't send herself, but came from her. Here's an example of one of the messages decline messages User B sent back to User A:

Sent: Monday, 3 December 2012 12:54 PM
To: User A
Subject: Declined: Group Meeting
When: Occurs every 2 weeks on Wednesday effective 26/10/2011 from 9:30 AM to 10:30 AM GMT+0800.
Where: # Boardroom



Is anyone able to shed any light on this? I've been pulling my hair out trying to track down the source.
