Anniversary update failed in the Safe_OS

Six attempts, six failures. I'm trying to install the Anniversary Update on my computer, after trying the upgrade assistant, followed by media creation tool, and a USB install created with the media creation tool. My PC is an I7-4790 3.6ghz on a Gigabyte GA-Z97X Gaming7 motherboard. I'm currently running Win 10 ver 1511, build 10586.494 my C: drive is a Samsung 950 M.2 500GB SSD.


Each failure results in the same error message:"0xc1900101-0X20017 The installation failed in the SAFE_OS phase with an error during BOOT operation"
